Employee Number	17	Y		Print Employee Number

Year to Date	18	Y		Print the total of contributions calculated for the financial year to date
		N		Suppress year to date total
Total to Date	19	Y		Print the total of all contributions calculated to date
		N		Suppress total
Standard Rate	20	Y		Print the percentage by which contributions are calculated, or the standard contribution dollar amount
		N		Suppress the percentage or standard amount
Birth Date	21	Y		Print the employee's birth date
		N		Suppress the birth date
Termination Date	22	Y		Print employment termination date, if applicable
		N		Suppress termination date
Print Control 1	23	Y		Print only employees with year to date figures. This overrides option 18 if selected
